Health Services Care Coordinator (Posted 7/25/24)

Purpose: The Health Services Care Coordinator receives intake calls, provides information, schedules visits, and manages referrals to both inter- and intra-agency resources. The Care Coordinator works collaboratively with the agency staff to ensure that clients’ needs are addressed, and that supportive documentation is completed in a timely manner. This work is done in support of Enable’s mission, and within the guidelines of Enable’s policies and procedures.

ESSENTIAL DUTIES

Outreach Activities
Work within the community is designed to identify at-risk persons, determining their eligibility and need for health, wellness, and social support services, as well as care management. Additionally, plan, organize and facilitate health fairs in underserved areas.

Screening
Conduct a preliminary assessment of the client’s circumstances and resources. This assessment may be by phone or in person.

Home Visits
Perform home visits to conduct assessments of clients for safety, support systems, physical and emotional well-being, financial stability, substance abuse, etc., in accordance with and support of Enable’s mission to provide monitoring of individual/familial situations and provide appropriate and effective intervention for their well-being if needed.

Service Arrangements
Contact and/or put the client in touch with both formal and informal providers to arrange services specified in the plan. This may include, but is not limited to, support provided to clients during the application process for food assistance, voucher assistance, transportation needs, etc.

Ongoing Monitoring

  • Provide case management and care coordination; building effective client relationships; assisting persons with processing information and resolving personal issues; providing education and linking clients to local community resources; managing crises, etc.
  • Modify the care plan as needed.

Documentation

  • Compile monthly reports and make available to the Executive Director, per guidelines provided
  • Maintain updated records and client records that support Enable’s need to demonstrate the efficacy of the agency’s health services to funding providers and the community at large.
  • Collaborate with Enable staff to update the agency’s, especially Safety Net’s, policies and operating procedures, as needed.

Outcome Measures

  • Make home visits as required by workload, within the scope of this position, and at the direction of the Executive Director.
  • Complete outcome measurement tools that demonstrate Enable’s ability to carry out its mission and support the documentation requirements from funding providers.
  • Orient, manage and supervise student interns, volunteers and others per agency policies and procedures and/or within academic curriculum guidelines.
  • Collaborate with other Enable staff members and with outside resources to achieve the goals established for each client.
  • Respond to inquiry calls; assist each caller in a professional and compassionate manner and link to community resources.
  • Communicate with referrers and other community resources as necessary to improve knowledge of community-wide services to better serve Enable’s population.
  • Meet regularly with the Executive Director to review client status and to ensure that services are being provided as planned.
  • Contribute to staff meetings in a productive manner and participate in community-based meetings and/or elder care events as requested, to develop relationships with referrers and other community resources, and for the good of the agency.
  • Gather necessary data and calculations for Grants Coordinator for major grants, 6-month and annual reports for funders such as United Way of Madison County, Alpha Foundation, etc., as directed by Executive Director and/or Grants Coordinator.
  • Performs other duties as assigned by Executive Director

QUALIFICATIONS

Education/Experience

  • Bachelor’s Degree, Master’s Degree preferred, in Social Work or Health Care Management
  • Minimum 1 year experience in a health care or community services environment
  • Previous volunteer or paid work experience in a non-profit agency preferred

Knowledge

  • High level of proficiency or expert knowledge of Microsoft Office, FileMaker Pro, and Mac-OS systems
  • Proficient knowledge of how to operate standard office technology (computers, teleconferencing technology, projectors, copiers)

WORK SCHEDULE
Regular office hours are Monday through Friday from 8 AM to 4:30 PM. Work schedules may need to be adjusted from time to time to meet the needs of the agency and the clients it serves, at the request of the Executive Director.

JOB POSTING FOR ENABLE EXECUTIVE DIRECTOR

Enable Madison County is seeking a dynamic leader who is passionate about the Enable mission to serve as the Executive Director. The Executive Director will promote Enable’s mission and secure funds for the overall operation of the organization.  The Director will oversee the administration of the organization, collaborate with community and governmental organizations, provide outreach to elderly and homebound individuals, and coordinate with Enable’s Board of Directors and Executive Committee on agency programs and operations.

Key Responsibilities

The Executive Director will be an active participant in the broader Huntsville and Madison County community to help increase awareness of Enable and its mission, to promote partnerships and participation that help carry out the mission, to ensure future funding for the organization, and to be an advocate for aging populations and programs. This work is done in support of Enable’s mission, and within the guidelines of Enable’s policies and procedures, and includes the following:

  • Serving as a liaison within the social service agencies, United Way of Madison County, Area Agency on Aging, local hospitals, community organizations and federal government agencies, by networking with organizations, businesses, speaking on behalf of Enable, and serving on appropriate community committees,
  • Developing comprehensive marketing and development plans, carrying out fundraising activities, building relationships with supporters and stakeholders, seeking and applying for grants, and other activities in coordination with staff, board members, and volunteers.
  • Cooperating with community organizations to assure that Enable is effectively using resources in the community and to assure referral mechanisms are in place,
  • Providing consultation and advice to the Board of Directors in all matters concerning program development, implementation, and administration, and
  • Managing the office staff, including setting operational policies, hiring, training, and reviewing employees, and ensuring smooth operation of the organization.

Qualifications

  • Bachelor’s degree in marketing, public relations, business management or related areas.
  • Minimum 5 years of experience working with non-profit management and/or development.

Health Services Care Manager – RN:

Job Purpose:

The Health Services Care Manager carries out the care management process with clients by completing assessments, care planning, plan implementation, and monitoring. This requires that the Care Manager visits clients, communicates with clients and family members, and works collaboratively with resource providers, both inter-and-intra-agency. This work is done in support of Enable’s mission and within the guidelines of its policies and procedures. Health Care Manager collaborates with Health Services Care Coordinator to ensure that clients’ needs are addressed, and supportive documentation is completed in a timely manner.

Responsibilities

Outreach Activities

Work within the community designed to identify at risk persons likely to qualify for and need health, wellness, and social support services, as well as care management. Additionally plan, organize, and facilitate health fairs in underserved areas. Attend COA and Advisory meetings in person and virtual.

Screening

Conduct a preliminary assessment of the client’s circumstances and resources. This assessment may be by phone or in person. During Covid-19 most of the assessments were done over the phone in order not to spend too much time with the client for risk of infection.

Home Visits

  • Perform home visits to conduct assessments of clients for safety, support systems, physical and emotional well-being, financial stability, substance abuse, etc., in accordance with and support of Enable’s mission to provide monitoring of individual/familial situations and provide appropriate and effective intervention for their well-being if needed
  • It focuses on physical health, mental functioning, ability to perform activities of daily living, social supports, physical environment, and financial resources. Also, look at their medications to educate client on interactions and side effects of the medications.

Care Planning

  • Use the information collected during the assessment process to develop a plan.
  • Care planning requires clinical judgment, creativity, sensitivity, and knowledge of community resources.
  • Care managers consider the willingness and availability of informal caregivers to provide care, attempting to establish a balance between formal and informal services.
  • Clients and caregivers (where available) participate in developing the care plan.
  • Care planning is a key resource allocation process and a critical care management function.
  • We follow up with the client and family to see if their needs were met.

Community Outreach Coordinator:

Job description

Job Purpose:

The Community Outreach Coordinator supports all aging-in-place services through fundraising, marketing, and outreach efforts. The Community Outreach Coordinator is responsible for recruiting volunteers for assigned services to maintain this program. Coordinates fundraising and event activities with the Resource Development Committee and other subcommittees. Manages donor database for accurate tracking and reporting purposes. Organizes volunteers and donors for the Enable Christmas gift service. Creates content and manages the website, outgoing communications, and social media for the organization with the oversight of the Executive Director. The employee performs these duties by communicating effectively with volunteers, donors, and other staff members, being attentive to client needs, and managing time and resources efficiently. This work is done in support of Enable’s mission and within the guidelines of its policies and procedures.

 

 

Outreach Activities

Support administration, assist with marketing, fundraising, and public relations efforts and recruit volunteers.

 

Resource Development

  • Maintain, update and track contributions, mailing lists and donor/volunteer data in the database.
  • Support Enable’s Resource Development Committee in development of fundraising and special promotional event activities.
